What is the Social Care Network?

The Social Care Network, also known as SCN, helps connect Medicaid members with support for everyday needs that can affect health and well-being.

These needs are called Health-Related Social Needs, or HRSNs, and may include things like food, housing, transportation, utilities, care management, and other social or economic challenges.

Through the SCN program, eligible Medicaid members can be screened to identify unmet needs and connected with services or community resources that may help. The goal is to make care more complete by addressing not only medical needs, but also the real-life factors that impact a person’s health.
